The application is addressed to the Director of Vilnius nursery-kindergarten “Justinukas”. The form contains:
- the applicant’s name and surname;
- address, telephone number and email address;
- the subject of the application (the line “DĖL …”, meaning “regarding …”);
- date and place (Vilnius);
- the text of the request (the line “Prašau …”, meaning “I ask …”, followed by free text);
- signature, name and surname.
